Death Occurs
Appointment of Agent to Control Disposition Form
The form called "Appointment of Agent to Control Disposition of Remains" allows an
agent and two (2) successors to be specified to handle any and all arrangements pertaining
to the disposition of one's remains, including cremation, in accordance with Section 711.002
of the Health and Safety Code of Texas. This form completed and signed in front of a notary
eliminates our funeral home from having to get signatures from family members after a death
occurs in order for cremation to take place. The appointed agent or successor is able to sign
all necessary paperwork in the State of Texas.
Self-Intent Form
The form called "Statement of Intention to be Cremated" can be completed by someone
desiring cremation. This form must be completed and signed by a person of sound mind in
front of two (2) witnesses and a notary in order for it to meet crematory requirements in
the State of Texas. This form eliminates our funeral home from having to get signatures
from other family members after a death occurs in order for cremation to take place.
The original signed and notarized form is needed by the funeral home in order for it to be
used. The completed form can be kept in a place where family members or friends would have
access to it at the time of the person's death, or can be held at our funeral home in a
pre-arrangements file for that individual.
Self-Intent Form - Power-of-Attorney
The form called "Statement of Intention to be Cremated" can be completed for someone
desiring cremation by another who holds a Power-of-Attorney (POA). A POA is only in effect
while the Grantor is living. When death occurs, the POA over an individual's affairs also
dies. The POA must complete this form in advance of the individual's death for it to be
valid, and a photocopy of the original POA paperwork must be attached.
This form must be completed and signed by a person of sound mind in front of two (2)
witnesses and a notary in order for it to meet crematory requirements in the State of
Texas. This form eliminates our funeral home from having to get signatures from other
family members after a death occurs in order for cremation to take place.

Texas Death Certificate Information Worksheet
Sometimes the biographical information that is needed to complete the
Death Certificate is not readily available when a death has occured. This
form contains portions from the front and back of the Death Certificate
that must be filed in the State of Texas. This is the information that
will be needed by the staff at Aldine Funeral Chapel when the death
occurs. Completing this information in advance may make things easier.
Use no abbreviations, print legibly, and leave blank or mark "UNKNOWN"
all appropriate fields.
This form, completed in advance, can be kept on file at Aldine Funeral
Chapel for your convenience.

This form, executed in front of a Notary Public by the legal next kin,
allows someone else to sign all of the necessary funeral home paperwork
authorizing cremation.
Authorization Regarding Delivery of Cremated Remains
This form allows the authorizing agent to designate alternate
delivery instructions for cremated remains.
