This 75 y/o female presented with acute abdominal pain. She was afebrile, there was mild generalized abdominal tenderness without rebound and a questionable soft mass in the left lower quadrant, bowel sounds were slightly increased, WBC - 12,200/cc³, urinalysis - unremarkable. An abdominal CT scan was performed. Selected images shown above demonstrate a left lower quadrant mass and a "whirled" appearance to the region of the root of the mesentery, suggesting a mid gut volvulus. Surgical exploration demonstrated small bowel lymphoma with a 180º twist of the small bowel.
