You can order two ways - via mail and via fax.

1.  If you order via mail, simply print out my order form - fill it out completely, and mail it to my address off my website.  Check, money order, visa, and mastercard are accepted.

2.  If you order via fax, you must use my order form off my site - Simply print it out, fill it out completely with payment being either in visa or mastercard, and fax it to me here at 972-878-5460.  If you have problems with the fax machine (it happens), simply e-mail me and I will give you an alternate fax line.

I can assure you that if these steps are followed completely and correctly, it will help both you and I in the long run.  Thanks in advance for your cooperation during this transition. :-)
