There are now additional ways of sending this questionaire back to me. In addition to saving it as a .txt file, to send it as a .txt format attachment; and, copy it from the screen, to paste into an e-mail; I'm adding links which will automatically insert the questions into the body of your e-mail. These e-mail links will be in red. APPLICATION RULES 1) I reserve the right to deny any application, for any reason. 2) No one under the age of 18 will be considered. Applications from persons under the age of 21 will be accepted only under extraordinary circumstances. 3) The application must be voluntarily submitted by the applicant. No third party applications will be considered. RETAIN COPIES, although e-mail is generally reliable, messages occassionally get truncated or don't go through. Both of these (especially the application) take a lot of work and emotional energy to fill out; so, you don't want to have to start back over from scratch. All information provided by you, now or in the future, will be considered to be confidential and will NOT be released to outside parties without your consent, with the following exceptions: 1) admissions of murder, admissions of child molestation, or threats against persons or property must and WILL be reported to the relevant authorities; 2) some details of your case might be used for staff training or demonstration purposes, if these occur they will ONLY be done in such a way as to insure that your identity is protected. 3) a copy of the video tape, application, and consent forms will be kept for the purpose of mounting a defense, in the event you should later decide to try to either file charges or a lawsuit against us. In limited cases, a copy of this tape, with all of your identifying features masked, might also be used for training or demonstration purposes; but, under no circumstances will it be used in a manner which is likely to reveal your identity, for anything other than legal defense purposes. 4) if you would like to request a copy of this tape (such as to present to your victim as evidence of your remorse; or. "insurance against habit guilt") and are willing to sign that you have a copy as well, one will be made available to you. Include examples of self-sabotaging behavior, "internal dialog," etc. 12) How did you arrive at the point where you've decided to seek out corporal punishment as a way of resolving your guilt feelings? Questions 8 through 12 13) Write a personal history and description of your self-concept. (Who are you REALLY and how did you become who you are?) Include a detailed summary of the event(s) which resulted in the guilt which you are feeling. If you feel that you need to do so, you may change: names, places, and dates; but, you are urged to keep the material facts as accurate and thorough as possible. 14) What part did the use of alcohol or other drugs play in your situation? Are you willing to become actively involved in a substance abuse program? Why? / Why not? If you already are, how long have you been "clean and sober"? 15) What other avenues have you pursued in an attempt to resolve your feelings about what occurred? Questions 13 through 15 16) Do you have any history of psychological or psychiatric problems. If so, how long ago and what did they entail? 17) Do you have any history of involvement in "BDSM" types of activities? Have you ever found punishment, or the thought of it, "arousing?" 18) Have you ever been determined to be incompetent or unfit to make decisions for yourself? 19) Do you understand that the people working with you are not, and make no claims of being, mental health professionals? 20) Does the gender of the person(s) working with you matter? Why; or, why not? 21) Do you have any medical problems which might pose a threat to your life, if you were to undergo such a punishment? (Such as: very high blood pressure, a history of heart trouble, stroke, etc.) When was the date of your last complete physical exam? Did this include an exercise cardiology test? Questions 16 through 21 22) Do you understand that that an assumption will be made that, after going to such extreme efforts, it would not be in your best interest for the punishment to be stopped for any reason other than obvious medical problems, regardless of any requests or demand you might make that it cease? 23) Would you be willing to fully comply with written assignments, both before and after receiving such a punishment? 24) Are you willing to comply with any and all requests made of you which are deemed necessary to carry out the punishment you are requesting? 25) Are you willing to comply with any follow up recommendations which might be made, such as: drug and/or alcohol treatment programs, refraining from the use of alcohol and any other non- prescribed drugs, professional psychological counselling, making restitution to any victimes, community service activities, etc.? Questions 22 through 25 26) What is your understanding of flogging as a form of punishment and why do you feel that it might be appropriate in your specific circumstances? 27) What are you hoping to accomplish by volunteering for such a painful experience? 28) Would you be willing to sign waivers of all claims of mental, physical, emotional, financial, or other damages which you might incur as the result of our assisting you to deal with this guilt through the infliction of corporal punishment (in the form of flogging) and to waive all recourse to any civil and/or criminal remedies against all persons involved - including but not limited to claims of: kidnapping, false imprisonment, assault, battery, endangerment, or any other such charges? 29) If your best friend came to you with your situation, requesting your advice and assistance, what would your responses be? 30) If you were addressing the committee which determines our sentencing recommendations right now, how would you "present your case?" What would you have to say in your own defense; and, as your own prosecutor? 31) If you were sentencing: a) your best friend, b) your worst enemy, AND c) a total stranger, for these actions (under exactly the same circumstances), what would your sentencing recommendations be, and why? What do you feel that the appropriate sentence would be in your case, and why? If different from any of the above, why?
