Press 'P' on the title screen, then enter "FRUITBAT" for the password. On the main title screen when "Press Fire" is flashing, use the 'Left Cursor' and 'Right Cursor' keys to select the starting level. Any of the following keys can now be used during play:

Enter "CATFOOD" for the password for greets.
